Q: Does Premier Health Services accept insurance?
A: Premier Health Services does not accept any insurance.
Q: Am I required to have a referral to be seen at Premier Health Services?
A: No, while referrals are great and always encouraged, you are not required to have a referral to be seen at Premier Health Services.
Q: Do you take cash?
A: Chronic pain management services must be paid with a credit/debit card only. (no green dot, vanilla, or bluebird prepaid cards)
Q: Am I required to bring my medical records?
A: No, you are not required to bring them in, however if you do not it may drastically increase the time in which it takes to get an appointment. You will be asked to sign medical records request form and we will request your records.
Q: What am I required to bring for my visit?
A: You are required to bring in you valid TN State ID and a credit/debit card in your name.
